What it actually reaches
Starts downloads without a save dialog and reads your download history, including filenames, source URLs and where each file was saved. It does not read the contents of the files themselves.
Why an extension would ask for it
Media savers, bulk image grabbers and export tools need it to write files where you expect without prompting once per file.
How to judge it
A permission on its own says little. What matters is whether it fits what the extension is for: a note-taking tool that reads every page you visit is worth a second look, while an ad blocker that does the same is doing its job.
